This form enables customers to apply for Decent Homes Grant, Disabled Facility and Adaptations Grant.
Once you have completed this form, a Housing Solutions Officer will contact you to arrange to visit you at home. The Housing Solutions Officer will discuss your needs and work with you to decide the best way to help so that you can live successfully and independently in your home.
